| Abstract: | Swiss acute care hospitals are treating an increasing number of eldery, multimorbid patients with a high need of support. Together with relatives, they tackle a variety of tasks in order to cope with the illness. It is one of the nurses key tasks to support the elderly persons self-management abilities. Therefore, relatives feel better involved and experience less stress after the hospital discharge. The aim of this study was to explore challenges that nurses experience in integrating relatives into the hospital discharge process and how they cope with it. A content analysis of focus group interviews was conducted to generate implications for practice. An interprofessional approach to discharge planning and a family-centered approach to care can support nurses meeting the family’s needs. |
